What to Bring to Your Appointment
- Your insurance card
- Photo ID
- Physician referral forms, if required by insurance
- List of prescriptions and/or over-the-counter medication, including dose and frequency
- Information about your medical and surgical history
- Recent x-rays or relevant records
What to Expect
At your first visit, you will be asked to complete a patient information form including insurance data so please bring all insurance cards, numbers, and referrals with you. If you need authorization from your primary physician, please bring it with you.
For your convenience, you may download, print, and complete the
patient information form prior to your visit.
You will then be interviewed at length, asked to provide a urine specimen, and then be examined. If you speak only a foreign language, we would appreciate your bringing an interpreter to facilitate communication. We do speak some Spanish and Italian.
We make every attempt to keep you well informed about your problem and its treatment. If for any reason you feel unsure about anything, please do not hesitate to ask. Your questions will be answered fully.
Fees and Insurance
Fees charged by this office are comparable to those charged in this geographical area. We participate in most insurance plans including Medicare.
We will help you to complete any insurance forms. Patients are responsible for copayments, deductibles, and balances. In most instances, Medicare assignment will be accepted for office visits and lab tests. Annual deductible and coinsurance charges remain the patient’s responsibility.
If a referral from your primary care physician is needed please be sure to have it with you at the time of the visit.
If you change insurance plans, please notify us immediately.
If for any reason you are unable to make payment, rather than to allow your account to become delinquent, please discuss the problem with us. We will try to accommodate you and find a solution. If you have any questions regarding fees, insurance, or payment, please ask.
Hospitalized Patients
Dr. Schrager admits patients to the following facility:

This facility is equipped with the finest and most modern equipment available, including laser, MRI (Magnetic Resonance Imaging), CT scanning, etc. For questions regarding hospital patients, we would appreciate one spokesperson calling during afternoon office hours. The doctor is available at all hours for urgent or emergency questions.
